• Santa Ynez Valley People Helping People Santa Ynez Valley Aging Adult Behavioral Wellness Program - $99,590 Partners: Golden Inn & Village, Solvang Senior Center, Buellton Senior Center, Housing Authority of Santa Barbara County, Dial-A-Ride This program expands behavioral health services by adding a social worker or marriage and family therapist and interns to provide counseling and group sessions to seniors age 65 or older in the Santa Ynez and Los Alamos Valleys. A licensed-clinician or pre-licensed intern screens patients at intake for behavioral health issues, including cognitive or memory related illnesses such as dementia or Alzheimer’s, and provides individual services or a connection to classes and resources based on severity. In partnership with Dial-A-Ride, transportation services are available as needed, including to Golden Inn & Village, Solvang Senior Center, and Buellton Senior Center. Community Outreach Cottage Health hosts a variety of free and reduced-cost programs for community wellness. These programs offer Cottage employees the opportunity to share their expertise with the community and provide care for those most in need. Outreach programs include: • The Art of Parenting – Educating Pregnant Adolescents Through collaboration among Cottage’s education and women’s services departments along with the Santa Barbara Unified School District, a class called the Art of Parenting will launch in the fall of 2019 for local pregnant adolescents. The class is taught by two Cottage Health educators and focuses on: — Helping prepare teens for having an infant at home and the transition to parenting — Helping teens develop a social support system and connect them with community resources Our goal is to prepare students for parenthood and help break cycles that perpetuate intergenerational socio-economic disadvantage and reduce subsequent health-related issues. • Parish Nursing Our Parish Nurses provide service to community members at a variety of faith communities, senior housing facilities and homeless clinics. Nurses provide health assessments such as glucose and blood pressure checks, basic wound care, education, referrals, screenings and vaccinations. • Coast Caregiver Resource Center Coast Caregiver Resource Center, a program of Cottage Rehabilitation Hospital, supports family and other unpaid caregivers of adults with cognitive disorders and other disabling conditions. CCRC provides counseling, long-term care planning, educational programs, support groups, advocacy, legal consultations and respite care. • Community Case Management Cottage Health Community Case Managers monitor high-risk patients discharged from the hospital to the community. They provide resources and services for these patients and transport them to their primary care physician appointments as needed. The first visit is always in person and is followed up with weekly phone calls or, if needed, continued home visits for up to a year. Nationally Recognized for Environmental Sustainability UPDATED: The Nutrition and Environmental Services teams at Santa Barbara Hospital (SBCH) take a leading role in not only creating nutritious meals featuring locally sourced food, but developing exemplary practices for waste reduction, recycling and composting.
