G Recuperative Care Program for Patients Experiencing Homelessness NEW: To serve some of the most vulnerable in our community, Cottage Health has partnered with PATH Santa Barbara to offer healing, housing and hope to patients experiencing homelessness who are consequently at risk of poorer health outcomes. Now fully implemented, the program helps homeless individuals recover in a safe and supportive environment after hospital discharge. Goals include increased access to permanent housing and medical care, decreased hospital visits and access to job training. Under the program, PATH provides up to 10 beds for patients discharged from Cottage Health hospitals in Santa Barbara, Goleta Valley and the Santa Ynez Valley. Patients can stay in the program up to 90 days and receive support from an onsite nurse, a health navigator for help with addressing social needs and respite care monitors. H Everyday Heroes – Celebrating Employees Going Beyond Their Job Duties Through Everyday Heroes celebrations, Cottage Health recognizes employees who go above and beyond their normal job responsibilities to make lives better for colleagues, patients and patients’ families. Our heroes come from all departments and occupations. The one thing they have in common is the desire to do whatever it takes to help others. To honor our heroes for their high standards and efforts, Everyday Heroes receptions are held throughout the year at Santa Barbara Cottage Hospital, Goleta Valley Cottage Hospital and Santa Ynez Valley Cottage Hospital where each awardee is presented with a certificate of appreciation from the President and CEO, Ron Werft, as well as a personal gift. Awardees’ supervisors, managers and co-workers can attend to cheer on their colleagues. I Population Health/Employee Resource Connect/Patient Resource Connect UPDATED: Population Health is an initiative created to improve the health outcomes of our communities, reduce unnecessary illness and avoid more expensive health care. By offering grant funding and collaboration to local nonprofits, Cottage is able to help more of our neighbors live healthier and more productive lives. Employee Resource Connect – Created to address the needs of our employees. During health screenings, employees are asked to complete a quick, confidential survey to identify if they need support with food, housing, transportation or behavioral health care. Resources are offered to those who take the survey and request assistance. In conjunction, the new Employee Food Program provides $50 for meals on employee badges. Patient Resource Connect – The pilot program surveyed 796 emergency department patients to identify their need for support with food, housing, transportation or behavioral health care.
