Voluntary Short-Term and Long-Term Disability Plans Employees have a combination of state disability insurance, voluntary short-term and long-term disability coverage. • State Disability Insurance (SDI) is provided through the State of California SDI program. Starting with the eighth day of disability, the SDI program will replace a portion of an employee’s weekly salary, up to a maximum benefit amount that is established each year. Benefits continue for up to 52 weeks, as long as the employee remains disabled. • Voluntary Short-Term Disability Insurance may be purchased by an employee to supplement the benefits provided by SDI. Voluntary Short-Term Disability (STD) pays a benefit equal to 20% of an employee’s weekly earnings, to a maximum weekly benefit of $1,000. • Long-Term Disability (LTD) Insurance is provided by Cottage Health at no cost to employees. After an employee has been disabled and unable to work for 180 days, the LTD plan will replace 60% of monthly salary, up to a maximum benefit of $10,000 per month. As long as an employee remains disabled, benefits will continue until age 65, at which time retirement benefits can begin. Travel Assistance NEW: Through a contract with On Call International, employees can receive travel assistance wherever they may be. This is a 24-hour, toll-free service that provides a comprehensive range of information and referral services to assist employees with medical care situations and other emergencies encountered when traveling. The service also provides information about passport/visa requirements, foreign currency and weather that employees can access before they travel. Pre-Paid Legal Plan Employees may purchase legal service benefits for a nominal fee. Covered services include estate and advanced directive planning, adoption support, real estate transactions, personal injury claims, civil lawsuits and identity theft.
